Thank you for choosing to exhibit your projects at this year's Baltimore County 4-H Fair. On this page, you will find ALL of the forms/certifications that you must complete in order to be an exhibitor. The Baltimore County 4-H Fair will be utilizing the Blue Ribbon Entry System. It is important that you have all your forms that are pertinent to the classes you are planning on entering.

Any forms that need to be mailed to the 4-H office, please submit to Baltimore County 4-H, 1114 Shawan Rd, Cockeysville MD, 21030

If you intend to exhibit ANYTHING (animals, crafts, ag commodities, etc...) in the Baltimore County 4-H Fair, please complete the following:

  1. 4-H Liability Release & Informed Consent Agreement
  2. 4-H Youth Code of Conduct
  3. 4-H Health Form

 

If you intend to exhibit any ANIMAL in the Baltimore County 4-H Fair, please note the Maryland Department of Agriculture has released specific guidelines, available here in PDF.  In addition to the forms above, please complete:

  1. 4-H Animal Science Code of Ethics
  2. If you are leasing an animal, please complete the Animal Lease Statement & mail to the 4-H office by June 1.
  3. A Certificate of Veterinary Inspection (CVI) is available only through your vet & must accompany each animal to the fairgrounds.  Exempt animals are rabbits, cavies, poultry & equines.
  4. Livestock Registration Forms for beef, swine, sheep, lamb, goat & dairy must be completed and accompany the animal to tagging/weigh-ins/fairgrounds.
  5. Complete MDA requirements
  6. Maryland 4-H Quality Assurance
  7. Complete a Livestock self certification form

 


In addition to the ANIMAL forms, there are species-specific forms for Horse/Pony, Dog, Rabbit, Cat & Poultry.  Those can be found below.

 

If you intend to exhibit a HORSE/PONY in the Baltimore County 4-H Fair, please complete:

  1. Horse/Pony Registration Card must be sent to the 4-H office by June 1
  2. Negative Coggins obtained from your vet
  3. Self Certification Form should be completed and accompany the animal to the fairgrounds

If you intend to exhibit a DOG in the Baltimore County 4H Fair, please complete:

  1. Dog ID Card sent to the 4-H office by June 1st
  2. Proof of shots/immunization from your vet
  3. Proof of license

If you intend to exhibit a RABBIT in the Baltimore County 4-H Fair, please complete:

  1. Rabbit ID Card sent to the 4-H office by June 1st
  2. Self Certification Form should be completed and accompany the animal to the fairgrounds

If you intend to exhibit a CAT in the Baltimore County 4-H Fair, please complete:

  1. Shot vaccine record
  2. Proof of license
